Plane Likely Operated on Autopilot After Pilot Lost Consciousness, Leading to Crash That Claimed 4 Lives, Including Mother and Child: NTSB Report

Nearly two years have passed since a tragic private jet accident in Virginia that resulted in the loss of four lives. In response to the incident, the National Transportation Safety Board (NTSB) has published their final report shedding light on the details of the crash.

Although the investigation could not pinpoint a specific cause for the accident, it raised numerous questions that remain unanswered. Key details about the victims also emerged, all tragically affected by the event.

The individuals who lost their lives included Adina Azarian, 49, her two-year-old daughter Aria, Aria’s nanny Evadnie Smith, 56, and the pilot, Jeff Hefner, 69.

Report on the Pilot and the Incident

The NTSB’s comprehensive report detailed a plane crash that took place in June 2023, tragically ending four lives. According to the findings released on May 14, it is likely that the crash was caused by “pilot incapacitation due to loss of cabin pressure for undetermined reasons.” A contributing factor was identified as the decision by the pilot and owner/operator to fly the aircraft without supplemental oxygen.

The NTSB noted that the available information suggests that the occupants of the plane may have experienced hypoxia due to a lack of oxygen during the flight. Symptoms of altitude-related hypoxia can often be ambiguous and vary by individual. If left unaddressed, this can result in severe consequences, including death.

Timeline of Events Post-Takeoff

At the time of the crash, the plane carried Adina Azarian, her young daughter Aria, the nanny Evadnie Smith, and pilot Jeff Hefner. As the investigation progressed, it was highlighted that the NTSB was unable to determine the exact factors leading to the cabin pressure loss or whether it occurred rapidly or gradually.

Shortly after takeoff from Elizabethton Municipal Airport, the pilot established communication with Atlanta Air Route Traffic Control Center just before 1:15 PM on June 4, 2023. However, approximately 15 minutes into the flight, he became unresponsive to attempts at communication from the air traffic control.

According to investigators, it is likely that the pilot suffered incapacitation during the ascent to cruising altitude, leading the aircraft to operate under autopilot until it was no longer manageable.

Flight Behavior and Final Moments

After achieving an altitude of 34,000 feet, the jet continued at this height. Approximately an hour later, it veered off course from its intended destination in Long Island and flew over the Washington, D.C. area instead.

About two minutes prior to the crash, a team of United Air Force pilots received clearance to approach the aircraft, leading to a “sonic boom” that was reported by numerous individuals nearby. The pilots noted that the pilot appeared slumped in his seat, and there was no observable movement from the other passengers.

At 3:22 PM, the plane began an alarming right spiral descent and ultimately crashed in a wooded region near Montebello, Virginia, marking a devastating end to the flight.

Pre-Crash Maintenance Issues

The investigation revealed that in the weeks leading up to the crash, several maintenance personnel identified numerous issues, particularly with the pressurization and environmental control system, as well as a missing pilot-side oxygen mask. This raises concerns about whether these problems were rectified before the incident occurred.

The NTSB report mentioned a lack of evidence showing that particularly concerning issues had been resolved prior to the flight taking off, which indicates potential negligence in ensuring the aircraft’s safety.

Pilot Health and Medication

Notably, the report outlined that the pilot had pre-existing medical conditions, including high blood pressure and high cholesterol. He was prescribed medications that could potentially impair his ability to operate the aircraft. However, investigators did not find substantial evidence suggesting that the pilot was at a drastically heightened risk of incapacitation or that he had mismanaged his medication during the flight.

The NTSB concluded that the circumstances of the accident indicated that it was probable that all occupants of the airplane were incapacitated due to a shared environmental condition, likely resulting from the loss of cabin pressure.
